15A NCAC 18A .3206 RECORDS, HEALTH REQUIREMENTS FOR PATRONS- operator could not provide filled out records of clients for the past year. Records shall be kept for a minimum of two years and shall be made available to the department on demand. Blank forms are availble to view, and they do contain the required client's personal/contact information. Please make sure to have clients' records available for future inspections.
