Found Oasis 146 quaternary ammonium sanitizer in laundry room, but when product was dispensed (mixed with water), the chemical that came out had chlorine in it (Oasis 146 is probably not properly set up). No chlorine or quaternary ammonium sanitizer test strips were available to check sanitizer strength. The lavatory, its handles, faucet, and vanity shall be washed using an all purpose cleaner and rinsed daily if the room has been occupied. An approved sanitizer shall be applied to the lavatory and vanity using a method such as a wipe-on chemically-treated disposable sanitizing towel or equivalent (or sprayed on). The sanitizer shall be left to air dry. Sanitizing solutions shall consist of iodophors, quaternary ammonium or other chemicals (such as chlorine) that are equivalent to the action of 50 parts per million chlorine solution. A method or equipment for testing the sanitizer shall be available and used to insure proper strength. NOTE: Properly set up Oasis 146 quaternary ammonium sanitizer so it can be used for sanitizing bathroom sinks and counters. 1.0

7. In room 102 the sink at the vanity area needs to be re-caulked. In room 102 the fixtures on the sink are corroded. Fixtures were rusty in rooms 120, 104, 214,213, 119, All fixtures must be in good repair and easy to clean. 1.5
8. The Quat sanitizer dispenser was on site in the laundry room, but does not dispense any sanitizer solution.. An approved sanitizer method must be used to properly sanitize all lavatories and vanities. All the lavatories and vanities must cleaned and the sanitizer must be applied. The sanitizer must be allowed to air dry. There was no test strips on hand to test the strength of the sanitizer. Provide a test kit with a color chart to test the sanitizer solution. The item noted was a point deduction on the last inspection. 2.0
10. In room 102 the paint on the ceiling was peeling in the restroom. In rooms 213, 214,229 there was mold on the ceiling in the restroom. In several rooms 204,214,213,209,119,225 there were dust bunnies on the walls and ceilings. All floors, walls, and ceilings must be in good repair and easy to clean. This is a repeat item. In room 119 there was one wall mounted light fixture that was not safe the light bulb was hanging out of the fixture and was not safe. 2.0
20. Found the air conditioning filters in all rooms checked to be very dust. All ventilation must be clean and in good repair. 1.0
25. In room 102, 104, 225 there were stained pillows. In room 225 there were hair pins in the bed that was suppose to be clean. 2.0
26. In all the rooms checked the carpets need to be cleaned. In the rooms that were checked the ceilings and wall need to be cleaned to remove the dust bunnies, dried food stains and other unknown stains that were on the walls. This is a repeat item. 2.0
27. The chairs and couches throughout the facility need to be better cleaned In room 209 there were drawers that were missing from the furniture. In rooms 102, 104,214,213,119,225 the mattress were stained. There was a hole in the mattress in room 214. In several of the room the freezer parts or the refrigeration units needs to be defrosted. 2.0
28. In room 102 there were a large amount of drain flies present. There were also fruit flies present. Take steps to eliminate the pest problem. 2.0
31. The linen carts were unlabeled. All carts must be labeled clean or dirty to designate the intended use. There was one cloth linen basket that was for clean linen that was heavily stained. This is a repeat items from the last inspection. 3.0
General Comments
On the top level between rooms 221 through 228 the threshold needs to be re-caulked there is a large gap between the door of the rooms and the threshold.
There was a strong mold smell in room 229. Started to inspect the room but noticed luggage and found that the room was still occupied. Left out of the room once noticed the room was still occupied. The renters were not in the room during the time it was entered by myself, the housekeepers and Brandon.
In the ice machine that was in the closet the light was not working. Provide a working light bulb.
The owner provide documentation that the correct carbon monoxide alarms were ordered. The ones in the rooms were not a dual battery alarms.
There was also in a sanitizer bucket bud lite and other beverages on ice in the laundry room.
Do not remove or obstruct the grade card call (919)796-7988 for a reinspection.
