The 2nd floor ice machine drain pipe needs to be air gapped, the drain pipe was down in the floor drain. Raise the drain to at least 1 inch above the floor drain.

Some chemical bottles were mixed up, disinfectant had been put into some of quat sanitizer bottles, do not mix chemicals. Some chemical bottles were not labeled, label all chemical containers.
