Repair cracked and chipping plaster ceiling in the basement. Basement floors require cleaning of mud residue caused by flooding. Replace damaged and missing floor tiles at the top entrance to the stairs to the sixth grade hallway. (Trip hazard) Reattach loose base tile also on the stairs to the sixth grade hall. General Comments
Replace missing paper towel dispenser in boys locker room bathroom #1707. There is a strong sewer gas odor in the fieldhouse boys bathroom #1905. The floor drains in this bathroom should have water poured into the drains regularly by cleaning staff. When the floor drains have dried out the empty pipes provide an access/entry point for sewer gas into the bathroom. Please address. The basement area is flooding frequently during heavy rainfall. The basement is in need of a sump pump and a drainage system is needed in the adjacent earthen crawl space. These mitigations are needed to prevent the saturation of the crawl space soil and the flooding of the basement. Repairs are also needed to prevent the possible deterioration of the building foundation in this location and to prevent mold growth in the basement. School facility sanitation inspection completed.

5. Boys bathroom 1315 has two hand sink faucets with sensors. Neither one of the sensors could be activated. They may need to be replaced. There may not be a functioning hand sink in this bathroom at this time. One of the faucets at the hand sink of the girls bathroom next to 1315 was torn out. Repair the faucet. 15.0
6. The carpet in the majority of classrooms is stained and may need to be replaced. Room 1513 smells musky. Recommend removing the carpet. Repair damaged wall corners in 1618. The paint on the back door of room 1502 is very flaky. The glass exit door next to room 1713 is heavily damaged and covered by a wooden panel. Repair the door ( the building could be entered by simply lifting the wooden panel ). 15.0
7. One of the metals panels on a set of lockers in the 1600 hallway has come loose. Debris is building up behind the panel. 10.0
8. The flat light fixtures in the media center have dust and dead insect accumulation. Clean the fixtures. The stage curtains are stained and need to be cleaned. The base of the metal panels under the windows needs cleaning in almost all classrooms. Lots of dust and spider webs build up along the base. 20.0

11. Found mouse droppings in the cabinets in 1413. Live small roach in the boys locker rooms. Note : Teachers have numerous types of cleaning chemiclas and disinfectants sitting on the tables. Once the students return on Monday, all chemicals must be stored or in a locked cabinet or in a cabinet 5 feet off the ground. 10.0
General Comments
Note about food sales at concession stands : Only individually packaged foods can be sold. If you open a food package, you need a permit. You may heat individually packaged items like corn dogs, but you cannot open the wrapper.

3. Comment: Adjust the pressure on the fountains by the 1600 hall exit to the gym. 0.0
4. B) Clean and/or repair the toilets with plastic bags over them in the 1300 and 2300 hall girls' bathrooms. Repair or replace the out of order urinal and reset the toilet in the handicapped stall in the boys' locker room. 20.0
5. A) The handsink stations in the 1300 hall boys' bathroom and 2300 girls' bathrooms need to be repaired or replaced, the counter tops on the top of the sink stations are loose in both bathrooms and the faucet assembly is loose in the boys'. Replace the missing faucet aerators on 2 sinks in the 1500 hall boys' bathroom. The faucet on the middle sink in the 1500 hall girls' bathroom is not working, repair or replace. 15.0
6. The linoleum flooring throughout the school needs to be cleaned and/or stripped and waxed. The carpeting in the majority of the classrooms in the school needs cleaning. Replace the moisture damaged ceiling tile in room 2301. Repair the peeling paint in the outside breezeways. 15.0
7. A) The gym storage closets need to be cleaned out and organized, shelving or rolling dollies/carts may be needed. Old/unused or damaged equipment needs to be removed. B) There is storage on the floor in the gym storage closets, auditorium stage storage, and the special needs classroom bathrooms; storage must be at least 15 inches above the floor or arranged in a manner that facilitates cleaning or is easily moveable. 20.0
8. Comment: Room 1418 has a strong mildew smell and the AC unit filters are very dusty, change or clean filters; there were no obvious leaks at the time of inspection but this room needs to be monitored for moisture. 0.0

3. All drinking founatin mouth pieces need cleaning. Adjust the water pressure at the fountain in the chorus room, the fountain outside of room 1303, and one of the fountains on the way to the cafeteria. One fountain near the cafeteria on 1200 hall has a small leak ( water runs constantly ). 20.0
5. The hand sink faucet in the boys bathroom in the library is corroded. Replace the faucet. 15.0
6. Paint is flaking off the metal beams and doors. The carpet is many classrooms is stained. The floors need more attention in harder-to-clean areas like the corners around lockers. Dust and debris are building up. Clean the floors around the cooking equipment in the small kitchen in the 2 story building. 15.0
8. Dust is building up on the duct work in the gym, the stage curtains, and the stage lighting fixtures ( and other high fixtures in the auditorium ). Clean the windows including the space above the windows ( spider webs; dust and dead insects on the windows and blinds ). The HVAC unit in 1429 is leaking. 20.0
11. Saw a number of large roaches in the gym storage room and trailer 4. Saw drain flies in the girls locker room. Note : Windows that are being used for ventilation should be screened to keep insects from entering the building. 10.0
General Comments
GC # 7 : The shelving in most storage rooms is only about 2 inches above ground level. This allows for debris to accumulate under the shelves, but it is not high to allow for floor cleaning under the shelves. Raise the bottom shelves or block the open space under the bottom shelf. Sand and paint the storage shelf in 1602.
