This center toilet leaks on the flush valve pipework and has poor waterflow for flushing. Repair toilet in the bathroom for classroom #602. This toilet has poor water flow for flushing. Repair clogged and nonfunctional toilet in the bathroom for classroom #508. Repair nonfunctional toilet in the bathroom for classroom #511. Repair toilet that is leaking on the flush valve in the center toilet stall of the boy`s bathroom on the orange hallway. Repaint chipped bathroom walls in classrooms #507, #511, #603, and #608. A cover plate is needed to seal hole in wall over sewage clean out access point in the end stall in the girl`s bathroom on the red hallway. Repair roof leaks in classroom bathrooms for villas #6 and #13. The roof leaks in the bathroom for villa #13 are major. The ceiling tiles in this bathroom are stained and are on the verge of breaking apart from water damage. Priority repair and replacement of water damaged ceiling tiles in both bathrooms is necessary. Replace missing ceiling tiles in the staff bathroom in room #211. Replace permanently stained floor tiles that are present in class bathrooms #309, #311, and #317. Class bathroom floors on the #600 hallway had build-up and residue and do not appear to have been cleaned recently by the nighttime cleaning contractors. The walls are chipping in the two bathrooms in the special needs class kitchen. These bathroom walls require repainting. 50.0
5. Build-up and residue were present in the classroom sinks on the #600 hallway. It appears that these classroom hand sinks are not being cleaned regularly by the nighttime cleaning contractors. Repair nonfunctional handwash sink in the boy`s bathroom on the orange hallway. Reattach loose handwash sink to bathroom wall in villa #10. 15.0
6. The tile floors in classrooms on the #600 hallway have residue build-up and appear to not be getting cleaned by the nighttime cleaning contractors. Repair roof and ceiling leaks in the following school locations: in classroom #605, in classroom #608 in the art class stockroom #501B, in the office health room #111, and in classroom #311 on the lower skylight ceiling and upper wall by the class bathroom. Replace water-stained ceiling tiles and repair water damaged ceiling and wall materials in these leak locations after the sources of the leaks have been fixed. Replace broken door cover over recessed floor plug in the media center. Repair is needed to prevent possible trip and fall hazards. Repair loose railings on the entrance ramps for villas #9 and #13. Priority repairs are needed for safety. Both ramp railings are so loose at the entry that the railings could possibly collapse under weight creating the possibility of injury. The hand railings are also very splintered on the entrance ramps for villas #9, #12 and #13. Repairs for safety are needed. The floorboard is split on the deck at the top of the steps for the emergency exit of villa #5. Priority safety repairs are needed since this floorboard could possibly collapse under foot and cause injury. Replace water stained and missing ceiling tiles in classroom #320 caused by a prior leak. 15.0
8. Repair nonfunctional wall lights above sinks in the bathrooms for classrooms #605 and #510. Lighting is very poor to nonexistent in these class bathrooms. Dead bugs from prior pest control treatments were present in the ceiling vents of class bathrooms #505, #506, and #509. These vents should be cleaned out to remove the dead bugs. Reattach loose ceiling light shield covers in rooms #511 and #317 and in the hallway entry to room #317. 20.0
11. The courtyard area has not been mown. The grass in this area is over knee high. This area requires mowing right away to prevent pest issues in the adjacent school building. The monkey bars on both the 3-5 playground and the k-1 playground are extremely loose in their overhead support brackets. Priority repairs for safety on the monkey bars are required to prevent possible collapse of the monkey bars under use and possible injuries. Administration may want to consider stopping the use of the monkey bars by students until repairs have been completed. Repair torn ground cover padding around the climbing block structures on the 3-5 playground. Repairs for safety are needed to prevent trip and fall hazards. 15.0

Repair broken latch on the kiln lid in the art class. The kiln lid cannot be properly sealed due to this broken latch.

General Comments
Class supplies stored in the bathroom for villa #6 should be removed. Bathrooms are not clean storage locations. Class supplies stored in class restrooms are also an impediment to the cleaning and repair of these facilities. Repair loose support brackets for monkey bars on the playground behind the modular buildings and on the main playground by the cafeteria. Priority repair to maintain the safety of this equipment is necessary. Remove and discard broken mirror stored in the kiln room #501-A of the Art class. The mirror should be discarded A.S.A.P. to prevent possible safety hazards.
