4. Repair two urinals that have clogged drains in the boy`s bathroom adjacent to the kindergarten hallway. Repair broken flush valves on two toilets in the 2nd stall and in the end stall of the girl`s bathroom in modular building/trailer #3. These toilets do not flush. Repair toilet in the end stall of the boy`s bathroom on the cafeteria hallway. This toilet has poor water flow for flushing. Replace closing arm that was knocked off of the entry door to the boy`s bathroom in modular building/trailer #3. Priority repairs for safety are necessary due to sharp pieces of jagged metal left on the bathroom door exterior from the missing closing arm. Replace water-stained ceiling tile around air conditioning vent in girl`s bathroom on the cafeteria hallway. Verify that any leaks that caused this stained ceiling tile have been fixed. 35.0
6. Replace missing ceiling tiles in the gym entry lobby and in the kindergarten hallway at the entrance to the girl`s bathroom. Replace rotted, deteriorated, warped, and loose floorboards on entry ramps and decks on Trailer #1, Trailer #2, and Trailer #3. The loose and warped floorboards on the ramps and decks create the potential of trip and fall hazards. The deteriorated floorboards also pose a possible risk of collapse under foot. Priority repairs for safety are needed. The floorboards on the edge of the deck entry into trailer #1 and on the ramp and deck of trailer #2 are especially deteriorated and in need of repair. The front ramp on trailer #3 is the side requiring repair of the loose and warped floorboards. 15.0
7. Class supplies including dolls, books, and pillows were stored in the pre-k class bathroom on the diaper changing table. Class bathrooms are not clean storage locations. Class items stored in bathrooms can become a method of transfer for communicable disease pathogens including norovirus and stomach flu. Only items related to diapering and bathroom use should be stored in class restrooms. The storage of class items in class restrooms is also an impediment to the cleaning and repair of these facilities. The class storage should be removed from this bathroom A.S.A.P. 10.0
11. The wooden wall adjacent to the instrument garden at the kindergarten playground has exposed nail points jutting out on both sides. There are also exposed metal brackets with sharp edges on this wall. The exposed nail points and sharp metal brackets on this wall pose potential injury hazards. The wall should be repaired or removed from premises. The large wooden chair in the cafeteria courtyard also has exposed nail points and also requires priority repairs for safety. Remove rotted and broken picnic table that is adjacent to the kindergarten playground from the school premises due to safety concerns. There is also a broken wooden chair adjacent to the instrument garden that should also be disposed of to prevent safety hazards. There is a large dead tree limb hanging over the walking/running track at the 4th and 5th grade playground. This dead tree limb should be removed A.S.A.P. to prevent a possible safety hazard for students and staff. The top of the plastic border wall by the sidewalk leading to the track for the 4th and 5th grade playground is broken. The broken plastic top of this border wall has sharp edges and could be an injury hazard. Repair or replacement of this plastic border wall for safety is necessary. Secure and fasten loose side border railing on steps of the 4th and 5th playground structure at the back end of the track. 15.0
General Comments
Cleaning chemicals including Clorox wipes and aerosol spray cans of Lysol were left out in areas accessible to students in the pre-k classroom, in the pre-k class bathroom, and in classroom #3141. Cleaning chemicals labeled as keep out of reach to children must be stored in closed or locked cabinets, closets, or stock rooms only accessible to staff. The diapering changing table in the special needs class kitchen #3153 girl's bathroom has a torn covering. Changing tables must be waterproof and nonabsorbent to properly disinfect the table surface between use for students. This changing table requires replacement.

General Comments
Repair or replace torn sunscreen over pre-k playground equipment. Cleaning chemicals and an aerosol spray can were left out on a cabinet top in the pre-k class #1106 bathroom. Aerosol spray cans of paint and other aerosol products were left out on counters and shelves in the art classroom #1102. Chemical products labeled as hazardous and pressurized aerosol cans should be stored in areas that are only directly accessible to staff such as a locked cabinet, closet, stock room, or desk drawer. The chemicals in the areas listed should be moved A.S.A.P. to an area where only staff members can gain access to the chemical product. Classroom items including plastic bins were stored in the pre-k class #1106 bathroom. Class bathrooms are unclean storage locations. The storage of classroom supplies in a class bathroom is an impediment to the cleaning and repair of this restroom facility. Only items related to diapering or bathroom use should be stored in class restrooms. Please remove these supplies from this class bathroom. School facility sanitation inspection completed.

11. Many mice dropping seen in every classroom that was visited. Modular units panel was off at girls hand sinks, a lot of dropping back there on book cases, window seals, floors. Roach droppings seen in class rooms. Ants present in a few classrooms. Hole in floor at fire alarm door. 20.0

Recommend these classrooms in the modular unit should not be used by students until rodents, ants, roaches are gone. Clean up all droppings. I recommend storing all aerosols, white board cleaner, white out and any chemical that says warning keep out of reach of children in cupboards or out of reach of children in the classroom.
