Repair leaking and nonfunctional toilet in the second stall of the modular building girl`s bathroom # M612-03A. Repair nonfunctional toilet in the 200-hall girl`s bathroom. 35.0
5. Repair clogged hand wash sink drain in the special needs/pre-k class kitchen bathroom #117-A. Repair nonfunctional hand wash sink in the girl`s hall bathroom #202. Repair clogged sink drain in the art classroom #227. Repair leak on the third hand wash sink faucet in the girl`s bathroom on the 400-hall. Repair nonfunctional hand wash sink faucet in the boy`s bathroom on the 400-hall. 15.0
6. Repair clogged drainpipe in the wall behind the washing machine in the special needs class kitchen #117. This clogged drainage pipe is causing leaks in the wall and prevents the washing machine from draining after use. Priority repair is required. Cracks between the cafeteria dining room wall and the adjacent floor should be sealed and repaired. These cracks are present along the base of the cafeteria wall with windows. Multiple ceiling leaks are present in the 300-hallway at the entrances to rooms #301, #302, #303, and #305. Ceiling leaks are also present in classroom # 303. Possible mold stains are present at these ceiling leak locations. The sources of these ceiling leaks must be fixed, and the water/mold-stained ceiling tiles should be replaced. The exterior amphitheater stage has deteriorated wooden floorboards. Holes are also present in this stage floor. Priority repairs of this stage floor are required for safety. Trip and fall hazards and injuries are a possibility if this stage is not fixed. If repairs cannot be completed the amphitheater stage should be removed from the school premises. 15.0
8. Repair nonfunctional air conditioning system in the gym. Priority repair is needed to prevent interior moisture condensation on the gym floors, walls, and ceilings. Repair of the air conditioning system is also a priority for the comfort and health of staff and students. Major amounts of dust residue are covering the ceiling vents over the gym stage. Cleanup of these vents is required. The air conditioning system vent filters for the gym stage area also should also be replaced. 20.0
11. Replace uneven ground tiles on the front playground. These loose and uneven ground tiles pose a trip and fall hazard. Possible injuries are a concern. Priority repair is needed for safety. Repair broken interior of slide on the front playground structure. There is a hole on this broken interior of the slide with sharp edges present. These sharp edges could create possible cuts, punctures, or wounds if not repaired. Priority repairs for safety are needed on the slide. Administration may want to close off this slide from use by students as a possible safety option until repairs can be completed. Repair loose spiral climbing post on the front playground structure. The anchor of this loose climbing post should be fixed into a secure position to allow students to climb safely. Secure loose base step on stairs of pre-k playground structure. Repair or replace broken ball catch on post on the back playground. Pre-k playground equipment is covered in dirt buildup and residue and requires a pressure wash cleaning. 15.0

These vents require cleaning by a Wake county school system maintenance specialist due to vent screens being bolted in place. This is a repeat cleaning request. Bathroom ventilation was first noted for this problem in the 2018 sanitation inspection and has not been addressed. Cleaning of these vents is needed A.S.A.P.

4. Several toilets are not flushing adequately or not working at all such as the girls rest room on the 200 hall. On of the Palace toilets is not working. 20.0
6. The wall is damaged at the urinals in the boys rest room in the palace..... The concrete floors settled in several class rooms and slope down on one end.....There are a lot of missing ceiling tiles in several halls....The rubber tiles in the fall zone of the front play structure are loose and should be repaired....The walls are rotten in trailer #3. The damaged panels need to be replaced. 30.0
7. The janitor closet in the palace needs cleaning. There is a bad odor from that end of the building that is coming from the janitor closet, the girls rest room and the boys rest room....There is dust on high ledges in many areas and on top of monitors, projectors etc. .....(B)There are not a lot of storage areas in the school the few that are here are packed full with things on the floor. All storage should be on shelves or racks high enough to sweep and mop under built so nothing can get underneath. 20.0

Clean areas around pet cages in facility. Ex: Guinea pig cages in gym service hall. Droppimgs were present around some cages. Cages and surrounding areas should be cleaned frequently. Children should wash hands immediately after handling class pets. Recommend that children do not handle class pets like turtles. Reptiles and amphibians present a greater risk of salmonella infection if touched. Repair loose step on playground equipment by modular classes. Replace damaged commode seat in boys gym bathroom.
