This sink has a clogged drain and is leaking beneath as a result. The adjacent dishwasher in this kitchen is also backed up with standing water as a result of the clogged plumbing lines. The plumbing lines for both sink and dishwasher should be unclogged. The sink disposal repaired, replaced, or removed. The dishwasher repaired or replaced. Repair roof and ceiling leaks in the following school building locations: in the main entrance lobby at the front office, in the cafeteria dining room #107, in room #200, in the media center lab room #102E, in the media center main room #102, and at the side entry/exit to the garden across from teachers lounge #122. Water damaged ceilings should be repaired and water stained ceiling tiles replaced after the sources of these leaks have been fixed. Repair and replace rotted and deteriorated siding beneath entrance doors on modular class buildings #702, #703, and #705. Replace damaged and deteriorated siding on modular class building #706. Seal hole in the side of modular class building #706. Ramp and deck hand railings, ramp and deck side railings, ramp and deck floorboards, and deck steps, on the modular class buildings are loose, deteriorated, rotting and broken. These deteriorated and loose ramp and deck floor boards, steps, and railings pose potential injury hazards. These safety hazards include possible trips, falls, and possible collapse of deteriorated floor boards and steps under foot. The railings are also deteriorated to the point that possible collapse under use is of concern. 7. Class supplies, teaching materials, and text books were stored in the custodial closet #407 adjacent to the mop sink. Custodial closets are not clean storage spaces. Splash and spray from the mop sink could potentially contaminate class items stored in this closet. Please remove. 10.0

General Comments
Fill in holes in paved court area beside playground for grades k-1. The holes in the pavement could create trip and fall hazards. Repair torn covering on steps on playground structure adjacent to the the pirate ship. Repair is needed to prevent trip and injury hazards. Reattach and secure loose support bar on ladder entry to the monkey bars that are at a higher level on the red playground structure adjacent to the school weather station. Secure and fasten loose green support post on elevated green railing for swinging and climbing on this same playground structure. Seal holes in ground covering around climbing wall ramps to prevent trip hazards. This play structure is also on the playground adjacent to the school weather station. Repair of playground equipment is needed for safety. Art class kiln room requires cleaning of debris and dirt from the floor and cobwebs hanging from the ceiling.

Red and green outside climbing gym has cracked plastic pieces on climbing curtain. Replace. Children''s fingers could get pinched between cracked plastic pieces and cause harm.

7. Classroom bathrooms must be clear of storage. Several classes in kindergarten wing and in modular class bathrooms had non toilet items stored inside. Ex: Class supplies, art supplies, artwork, educational materials, toys, snack food, toothbrushes, single service cups, and utensils. Food,utensils,and toothbrushes should never be stored in bathrooms due to potential for contamination. Ex: Modular # 13 and special needs modular class. 10.0
11. Cleaning chemicals, chemical surface wipes, aerosol cans, and white board cleaner should stored in areas not accessible to students. Ex: In locked cabinets, closets, drawers, or stock rooms. Several classes had these products stored in areas accessible by students. Use of these products should be done by staff only. Keep janitor closets locked at all times when staff is not present. Two closets that were not in use were found unlocked during inspection. 30.0
General Comments
Repair leak that is pooling water beneath modular class building. Repair kindergarten hall ceiling leak and replace water damaged ceiling tile. Keep mechanical rooms locked at all times when staff is not present. One mechanical room that was not in use with staff was found unlocked during inspection. Repair loose support rail on outside playground structure. In classes that have pets like guinea pigs staff only should clean pet cages. Pet cages should be cleaned frequently. Children should wash hands immediately after handling class pets. If children are brushing their teeth in classes their tooth brushes should be stored in a clean/dry location with designated individual caps covering the end of each child''s brush. Class sinks used for brushing teeth should be cleaned and disinfected before this type of use. Ex: Modular special needs class. Pudding cups provided to children with low blood sugar in med. rooms can not be saved for reuse once the product seal has been opened.
