14. Appropriate disinfectants were not available in any bathrooms throughout entire facility. Additionally, staff had no/incorrect/insufficient knowledge of disinfectant use, what to use, etc. After researching all of the products available at this facility, only ONE EPA approved disinfectant should be used for cleaning toilets, bathing equipment, lavatories, etc. and it needs to be available in ALL shared bathrooms at ALL TIMES. Discontinue using Orange Force for disinfection purposes and this is not its intended use. Instead, for ALL disinfecting, use the Oasis 299 disinfectant, item number 12146. No other cleaners available at facility are appropriate disinfectants. Additionally, only one, partially full container of this disinfectant was available at the facility, which leads me to believe that it may not be used nearly as frequently as is required (IN BETWEEN EACH PATIENT). Also, STOP USING BOTTLES LABELED FOR ONE CLEANER TO STORE ANOTHER CLEANER. THIS IS UNSAFE AND IS CAUSES CONFUSION OF WHAT CHEMCIALS ARE STORED IN WHAT BOTTLES. Acquire and use properly labeled bottles to stored only the chemical for which the bottles are labeled for, and USE THEM AS DIRECTED. 1.0

3. Clean the vents throughout the facility to remove the dust. There were several vents especially in Clarebridge that had mold build up on the vents. Take steps to remove the dust and mold. In the dinning room the paint is peeling in one spot above where the residents eat. In the same dinning room the caulking needs to be replaced around the ceiling area. There were gaps observed. In the hallway some of the baseboards are damaged and needs to be replaced. 1.0
8. In several rooms pink algae around the showers. In 106 the shower had pink algae growing at the bottom. In a few of the rooms the shoe molding needs to be reattached. In Clarebrigde the seat in the spa room is cracking and needs to be repaired. 1.0
