Remove class item storage from the shower stall in the special needs class shower room #305C. The shower stall must be kept clear to allow for cleaning and repairs. Repairs are needed on the nonfunctional elevator in the school. The elevator must be functional to provide access to the upper floor to disabled students and staff. Replace diapering table that has a torn surface cover in the special needs class kitchen bathroom #305B. The diapering table surface must be waterproof and cleanable. The torn covering prevents this. Replace torn rubber covering on handhold on support post on play structure in the back of building playground. Repairs for safety are needed on this handhold covering due to sharp exposed metal edges that could create possible injury hazards. Repair broken support chain on climbing platform on play structure on the back of building playground. Repairs for safety are necessary.

Improper chemical storage observed in classrooms, teacher’s workroom, unlocked classroom cabinets, classroom red emergency bags, unlocked adult restrooms and the unlocked receiving room. Chemicals labeled " keep out of reach of children" with no other labeling is to be kept 5 feet or higher off of the floor when not in use. These chemicals include hand sanitizer, disinfecting wipes, and some lotions. Chemicals with other warnings in addition to " keep out of reach of children" or “keep inaccessible to children” are to be kept locked. These would include anything in an aerosolized container and any cleaning chemical such as spic and span, washing detergent, Clorox disinfectant and mineral oil and any strong chemical in the receiving room. Also on spray bottle with a chemical inside was found in the locked custodial room 2302 unlabeled. Please label all chemicals. The receiving room door lock is in disrepair. Please repair or replace door lock so that toxic cleaners/chemicals are stored inaccessible to children.

Please re-organize the lost and found area so that cloths and bags are not accumulated on the floor. Educate teachers about labeling spray bottles that contain water (rooms 320, 2310). Stagnant water observed in playground equipment in the Pre-K playground. Please ensure water is removed from equipment before children uses it. Ground covering at the playgrounds (5th grader and Kindergarteners playgrounds) are worn/torn in need of repair. The Pre-K gate is in disrepair-the stake to the fence is not aligned with the fence stopper in the ground so that the fence closes properly.

General Comments
Implement moping floor in Pre-K nightly due to lunches being eaten there. Detail clean toilet in Pre-K room. Elevate all storage off floor at least 15" to facilitate cleaning. Strongly recommend increasing hot water temperature at handsinks on K/1st grade hall. Currently registering 64-68F. To encourage longer handwash time hot water can be turned up to between 80 and 110F. Temperature should not exceed 110F to avoid burn potential. Playground equipment looks good. Daytime custodian accompanied me initially. Ventilation and lighting looks good.
