Priority repairs for safety are necessary due to the risk of possible collapse of the awning. Children should not use the Pre-K playground equipment or play beneath or near the awning until it is fixed and is found to be structurally sound. Repair cracked interior surface of the slide on the playground structure on the K-2 playground. Repairs for safety are needed on the slide surface due to sharp pieces of broken plastic that pose potential injury hazards. Repair loose support post on spinner stool also located on the K-2 playground. Repairs for safety are needed due to the potential of the spinner support post breaking during use by students. School administration may want to consider closing off the use of the slide and spinner stool to students until safety repairs have been completed. 15.0

Repair clogged toilet in the last stall of the girls bathroom on the 4-5 hallway. Repair loose and damaged toilet stall support brackets in both the boys and girls bathrooms on the main hallway. Seal hole in ceiling above emergency lighting in the k-1 hall at room # 208. Repair chipped ceiling paint in the k-1 hall in this same location. Repair ceiling leaks in the media center at air conditioning vent, in classroom #211, in classroom #214 in classroom #215, on the access panel in the hallway at room #301A, at the exit signs adjacent to the custodial offices #124, in custodial office room #124A, and in special need class kitchen #117. Repair damaged wall covering above writing board in special needs class #115. Reattach loose green glass marker/writing board to wall in the 4-5 hallway . The board wall fasteners are loose. Priority repair is needed. The marker board is adjacent to the student hall bathrooms. Replace three damaged floor plug covers in the media center. Priority repair is needed to prevent trip hazards. Repair broken door seal in cafeteria fire door exit to the 4-5 playground. Replace missing plug cover on wall in the boys bathroom on the 2-3 hallway. There is an exposed sharp edge on this plug due to the missing cover. Repair broken door latch cover on the fire exit in room #216. Repaint chipped walls in classroom #218. 15.0

General Comments
Remove broken dresser cabinet from the special need class bathroom in room #117. The broken drawers and splintered wood on this dresser could create possible safety hazards for students. Remove items stored on the floor beneath the changing tables in the special needs class bathrooms. Bathroom floors are not clean storage locations. Classroom items and toys should be removed from the special needs class bathrooms. Bathrooms are not clean and designated storage locations. Only items related to bathroom use and for diapering or changing student clothing should be kept in class bathrooms. Mice droppings and a dead mouse were present in the drawers beneath the stove in the special need class kitchen # 117. The stove should be cleaned frequently of food debris that can attract pests. Recommend pest control measure be taken to prevent rodent infestations in this kitchen. Pest control should also be required to follow up on traps placed for rodents. The dead mouse found was in an old glue trap that had not been checked on for quite some time. A thorough cleaning of the stove should be completed A.S.A.P. The Pre-K playground area has not been mown and the lawn within this area is becoming overgrown. Mowing of this area is needed to prevent possible pest issues in the adjacent school classrooms. Repair loose metal band on support post of play structure on the K-2 playground. This band has sharp edges and should be removed or reattached to prevent possible cutting hazards. Priority safety repair item. Secure loose support post on blue spinner on the K-2 playground. Repair for safety is required. Replace cracked and bent support rods on canopy over the Pre-K playground equipment. Repair is needed on these damaged support rods to prevent possible collapse of the canopy. Remove or replace damaged and frayed cable from chimes on the 4-5 playground. Sharp exposed metal fibers on the damaged cable could cause possible cuts or punctures for students. Please repair A.S.A.P. Beetles were present in the bathroom of classroom # 208. Professional pest control measures for insects should be taken in this location.

Remove broken bench from school wide garden area. First grade playground is flooding frequently. This playground needs a drain pipe or drain way to carry away standing rain water. Repair nonfunctional dishwasher in special needs classroom kitchen. Teacher in classroom # 216 has stated that the air conditioning system in this room is not circulating and cooling properly. (Recommend a maintenance check on the air conditioning system in this room.)

3. The water pressure needs adjusting at the water fountains in Rooms 206, 208, 303, 2319, 2303 and at the tall water fountain by bathroom 2314. Only the front buttons work on the short water fountain by bathroom 2314. Water fountains must be in good repair and have enough water pressure to create an arc that prevents water from splashing out of the draining area and prevents mouth contact with spout. Please have the water pressure adjusted at the water fountains. 20.0
4. There is a small hole in the wall under the toilet tissue dispenser in one of the bathrooms that is located in the kitchen that is adjacent to the special needs rooms; there are cracked floor tiles in both of these bathrooms. Girls` bathroom 2314 has a stall that does not have a toilet tissue dispenser - it has been ordered. Girls` bathroom 107 and boys` bathroom 109 need painting; especially around the urinals in the boys` bathroom. Walls, floors and ceilings and all fixtures must be clean and maintained in good repair so that they are easily cleanable. Please patch hole and paint bathrooms. 15.0
5. Cold water is not available at the handwashing sinks in 102M (mailroom) and 2319 (5th grade classroom). Please submit a work order to have cold water added to these sinks. 0.0
6. Carpet stains were found in the media center, Rooms 204, 307, 310, 317, 2309 and 2319; a small piece of tile is missing from the floor in Room 2309.. There were water damaged ceiling tiles in Rooms 117, 124 (along with black mold), in the hallway by Rm 120, 206 in the classroom and bathroom, 214 and 2303. Painting is needed in Rooms 310, 2319, the fourth and 5th grade hallway and the railing/walls of stairwell S1. Please have carpets cleaned, damaged ceiling tiles replaced after checking for leaks, and painting done in classrooms/hallways/stairwell. 15.0
